Caregiver Homes is a subsidiary of Seniorlink, Inc. in Boston, MA. Founded in 1999, Seniorlink is dedicated to offering community-based care options for frail seniors. Our Caregiver Homes program continues to break new ground in providing alternatives to long-term care placement for families and their aging loved ones. Seniorlink’s dedication to helping seniors age with dignity has been a cornerstone of the organization since its inception.

As a statewide provider of Adult Foster Care in Massachusetts, Caregiver Homes now offers an exciting alternative to long-term care placement by offering individual, caring homes for seniors in need. Families now have the assurance that care will provided for their elder loved ones by full-time caregivers (including family members) paid to take in and care for seniors who require daily assistance with personal care, meal preparation and transportation. Regional Director

The Regional Director has management responsibility for multiple programs and supervises Program Managers within a geographic region. The Regional Director directs the implementation of all aspects of the Caregiver Homes program throughout the region, and oversees the intake and referral process, placements and ongoing case management of all clients with related support and credentialing of caregivers. The Regional Director is responsible for managing the regional budget, recruitment and hiring of staff, customer relationships and marketing activity, delivery of the program and quality systems. This position is supervised by the Executive Director and works closely with the Seniorlink Executive Management team.

Qualifications: Minimum of two to five years of management and operations experience across a broad scope of eldercare and/or disability programs. Master’s degree preferred with experience and understanding of clinical aspects of programming, community and service delivery systems. Ability to oversee and direct multiple programs, build a regional team and build customer relationships. Use of multiple technology applications required. Regional travel.

Program Managers

The Program Manager directs and supervises all aspects of the Caregiver Homes’ program in his/her assigned geographic region, including: budget oversight, recruitment and credentialing of staff and caregivers, training, delivery of program services and assessment of participant outcomes. The Program Manager ensures that all documentation is completed on a timely basis, including assessments, plans of care, daily notes, home visit summaries, etc. and completes reports required by contracted agencies. He/she also conducts periodic visits to caregiver homes as defined by program requirements.

Qualifications: Minimum of five to seven years’ experience in case management and program operations in the field of eldercare. Master’s degree preferred, or an equivalent of education and experience in eldercare service and program delivery. Knowledge of case management systems, community service and delivery systems required, as well as a background in program operations. Strong clinical and communication skills are essential with competent use of technology applications. Some car travel within assigned geography is required.

Half-Time Registered Nurses and Care Managers for the Boston and Lynn Areas.

These two positions work collaboratively as a case management team to assess clients, to establish and implement a Plan of Care, to conduct regularly scheduled Homes Visits, and to oversee client care. The team credentials caregivers and their homes, and provides ongoing support and training.

Qualifications: Minimum of two years experience in case management, care planning and assessment for elders/disabled adults in a health care, long-term care, social service or community setting. Registered Nurses must be fully licensed in Massachusetts and have recent experience in the direct care of the elderly and disabled. Care Managers must have a Bachelor’s Degree and license in social work, or an equivalent combination of education and experience in these or related fields. Bilingual candidates preferred. Knowledge of case management systems, community service and delivery systems for aging and disabled population required. Strong clinical and communication skills are essential with competent use of technology applications. Regular car travel within assigned regional geography is required.
