Our model of eldercare has taken solid and successful root throughout Massachusetts. The proof is in our partnerships with other healthcare providers and successful client placements with family and non-family caregivers. Our program complements existing home health, adult day health, hospice and other services to offer a solid and truly unique base of services for our clients. Through professional collaboration, we - as a team - can work with you to help our clients stay involved, at home and in the community.
Who are our clients?
Our focus is on providing care to clients with complex health issues, many of whom are nursing home-eligible. Recent program data show that 20% of our clients have 5 or more diagnoses noted by a primary physician on the pre-admission statement. For clients over age 60, 37% have dementia, 35% have a psychiatric diagnosis, 65% have hypertension, 33% have diabetes and 21% have emphysema.
On average, our clients require physical assistance with 5.2 out of 10 Activities of Daily Living, and for clients over age 60, 65% have some level of bladder incontinence and 42% have bowel incontinence.
Clients residing with Family Caregivers tend to age in place longer than those who reside with non-family caregivers, by remaining in the community and in our program. Discharge to more restrictive settings occurs with nearly four times the frequency when clients reside in a non-family home, whereas Family Caregivers are more likely to continue to provide care through the end of life for our clients.

How do we collaborate with other providers?
Many CGH clients enter our program after qualifying through the Frail Elder Waiver. The coordination of care for these clients requires ongoing communication with the Elder Service Agencies (Aging Service Access Points, or ASAPs) to ensure complete care without duplicating key service components. CGH also works closely with Programs for All-Inclusive Care to the Elderly (PACE) and Senior Care Options (SCO) providers throughout Massachusetts.

How do we use technology to monitor client care and compile client and caregiver data?
Our care model has integrated a case management approach with a web-based technology platform - Seniortouch™ - for clinical documentation and client care planning, as well as for tracking and reporting of data. We require that all caregivers submit a comprehensive Client Status Report for their client(s) through Seniortouch.
Caregiver Homes’ experience in Massachusetts is that the information received through our technology platform enhances each team’s ability to intervene quickly, and in turn, to communicate emerging concerns with external health care and social service providers. The results are rapid clinical response and improved client outcomes and satisfaction.
Leadership and guidance from our Medical Advisory Board
With partnership in mind, Seniorlink has also created its own unique and distinguished Medical Advisory Board. Our Medical Director and Medical Advisory Board members advise Seniorlink and Caregiver Homes on a wide range of topics, including how the organization can continue to meet the needs of the older and disabled adults through our clinical processes.
The Board advises on care policies, standards and treatment outcomes and provides practical and strategic insight as well as oversight for developing and maintaining best practices as the company expands and adopts meaningful program enhancements.
