Where to start with your family member


You are not alone.

Our one and only focus is to provide a community-based, option that combines care with a safe home environment. Our services are for you, too. Caregiver Homes prepares you for the full-time job of caring for a frail elder or adult, and supports you every step, every day.

How it works:

Step 1: Qualify your elder parent or relative.
To be considered a client and be eligible for Caregiver Homes' assistance, your elder parent or family member must meet these criteria:
  1. Be a senior citizen who has or is eligible for MassHealth or be a disabled adult (at least 16 years old with Mass Health benefits.

  2. Need help with either:
    One or more activities of daily living such as bathing, dressing, eating, toileting, transfers (getting from bed to chair or from sitting to standing) and walking.
    Managing behaviors like wandering, abusive or disruptive behavior or resistance to care.

  3. Need 24-hour care or supervision.
Step 2: Clinical Assessment
> You and your family member meet with one of our registered nurse team members for a complete client clinical assessment.

> The state must approve the client for participation in the program.

Step 3: Matching clients and caregivers

Many of our caregivers enter the program together with a family member or friend. We provide caregiver orientation and training to best meet each client's needs.

If you choose not to be the caregiver, we look for the best possible match for your family member’s needs and preferences. Caregiver Homes has placed many clients with non-family member caregivers with great success. Read some of our inspiring testimonials.

Step 4: Working with your case management team

Once you and your family member are living in the same home, or your family member has been matched with an appropriate caregiver, the caregiver becomes part of the case management team and is supported by the Registered Nurse, Care Manager, and Program Manager.

This support includes:

> caregiver training and ongoing education to meet the client’s needs
> periodic reviews of the care plan
> regular phone contact and monthly home visits
> review and monitoring of care through Seniortouch™, a Web-based reporting system that keeps the caregiver in touch with his or her team daily.

> Registered Nurse
> Care Manager
> Program Manager
> Seniortouch Communication Technology Tool
