Our model, known as Structured Family Caregiving, was the first home and community-based model in the nation to receive the National Committee for Quality Assurance’s (NCQA) highest level of accreditation for case management. It's an alternative to other models of care that rely on facilities or in-home workers such as personal care attendants (PCAs) or home health aides (HHAs).
Structured Family Caregiving offers guidance, financial assistance and overall support at every step of the way, helping caregivers feel more confident in providing care for loved ones.
