If a family member or loved one makes a commitment to care for you at home, we make a commitment to them. We stand alongside your caregiver, providing the support and assurance they need to provide you with high-quality care and the comfort of home.
Our unique model of in-home care begins with a dedicated care team, including a nurse and care manager, who get to know you and your caregiver and assess your specific needs. They work closely with you to develop a personalized plan of care, and build a collaborative relationship that is focused on providing you with the best care possible. Through frequent visits and ongoing communication, they help you and your caregiver solve day-to-day challenges and anticipate and prepare for what’s ahead.
In order to keep care teams informed and involved each day, your caregiver fills out a quick electronic note in our easy-to-use, Web-based, care-management system. These notes are monitored by care teams daily, to ensure they can help your caregiver maintain the highest level of quality care possible.
To be eligible for Caregiver Homes, you must be eligible for Medicaid, need 24-hour, live-in care, and require help with daily activities, such as:
